^ 24 Communities in Frontenac, Lennox and Addington Ontario Health Team (OHT) were developed as part of a collaboration with Queen's University Health Services and Policy Research Institute (HSPRI). The result is a set of boundaries defining communities, their names and IDs that cover rural Frontenac, Lennox & Addingtonsub-region (of the former Kingston LHIN), and respect Census 2021 aggregate dissemination area (ADA) boundaries. Of the original 26 ADAs, two sets of two ADAs were aggregated together to increase the combined populations to reportable levels. One of the 24 communities aligns with Tyendinaga Mohawk Territory and it is omitted from our data gathering and dissemination in accordance with Ownership, Control, Access and Permission (OCAP) guidelines which OCHPP honours (https://fnigc.ca/). The resulting 23 communities will be used for reporting.
Due to limitations, data may not always be provided in chart or map form. If you require a specific format not available on OCHPP, please contact us at healthprofiles@unityhealth.to
